Parents/Guardians will be required to bring the following items for Roundup an dRegistration:

• Birth Certificate
• Social Security Card
• Up to Date Immunization Record
• Two (2) Proofs of Residency (utility bill, driver’s license, etc.)
• Two Latest Consecutive Check Stubs
• SNAP Letter (with student’s name on it)
• SSI Benefits Letter
• Foster Care Placement
